- Broker-only authority not eligible
- Intrastate-only authority requires verified intrastate permit
- Executed Carrier-Broker Contract
- Insurance
- Auto Liability: $1,000,000
- Provider rated at least B+
- Cargo: $100,000
- Provider rated at least A-
